Diabetes: Self Management Program

Diabetes: Self Management Program

This program enables individuals with diabetes to learn self care skills that will promote better management of their diabetes treatment regimen.  A certified diabetes nurse educator, along with a registered dietitian, offer individual or group instruction to those persons newly diagnosed with diabetes or those who would like a review.

This comprehensive program covers:

  • Diabetes overview
  • Monitoring blood glucose
  • Nutritional management
  • Preventing, detecting and treating acute complications
  • Preventing, detecting and treating chronic complications
  • Incorporating physical activity
  • Integrating psychosocial adjustment to daily life
  • Goal setting to promote health and problem solving
  • Promoting preconception care, pregnancy and gestational diabetes, if applicable.
